Legal Question in Workers Comp in New Jersey

I was injured at work on September 17 2013 I received treatment and was able to return to full duty by December 2013. I recently started to have the pain again and reopened my workers comp case and when I seen the doctor and had an up to date MRI done he put me out of work. Should I be able to receive paid compensation for this ? My job had another insurance company send me FMLA papers for me to sign and I'm not understanding why I should have to fill out for FMLA if I'm on workers comp. can you help explain this to me I live in NJ...
