Legal Question in Workers Comp in New Jersey

I have been out of work with an injury for the last 3 years, have had 2 shoulder surgeries, and foreseeing a 3rd surgery within a few weeks.

Two questions.

1. My uninjured arm is starting to get weak and experiencing pain when I do certain movements. Can I file a claim for my uninjured over compensating arm?

2. Being I have been seen by 4 separate surgeons, how do I go about getting a letter for a SS claim, when every lawyer says they need my treating Dr to write a letter stating I will be out of work for the next 12months. I've been out of work with my injury for the last 3years... Do I ask the original Dr, or the last Dr I seen who wants to give me another surgery?
