Legal Question in Workers Comp in New Jersey

Workman's comp.

I fell in work and was injured. 1/8/08 I had surgery on my meniscus (knee). I am currently out of work on workman's comp. I go to physical terapy 2 X per week. Today the Dr. gave me a cortozone shot because the pain is not subsiding. I have no relief from that shot as of yet. What are my alernatives?

I do not feel that i can return to work in this condition. I am worried it might not get any better. What is needed for permanant disability? (if this would even apply) Would that be from my employer or the state? What about my health insurance that I carry thru my job? will i lose that? I do not know where to turn for answers, anything you can tell me will be greatly appreciated. FYI I am a 58 years old female.
