petition to move case from 1 district to another

I live in New Mexico. My daughter and her mother live and have always lived in NY. When i moved here to Alamogordo, NM, NM Child Support Enforcement was asked by NY Child Support Enforcement to take juristiction for collection purposes. I have moved from Alamogordo, NM to Farmington, NM. My support case is still in Las Cruces (home to Alamogordos Judicial District) some 400 miles from Farmington.

I am disabled and no longer drive.

I am having problems with support collection unit.

I want to petition the Judicial District in Las Cruces,NM to change juristiction to the Judicial District here in Farmington, NM. There is no objection from my daughter, my daughters mother or from the State of NY to this move.

I need to file a petition to do this. what form do i use?
