Bankruptcy Creditors List - Who do I list and who do i not have to list?

I will be declaring bankruptcy soon. I have about 12 credit card accounts the debts with whom i hope to discharge throught he bankruptcy. However, many of these card companies have already sold the debt to other credit agencies. My question is: do i have to include all the credit card companies in my list of creditors AS WELL AS the credit agencies they sold my debt to or can i just include the original creditors I established debt with initially? In other words, if my debt is originally with credit card A and they decide to sell that debt to credit agency B, do i have to include both credit card A AND credit agency B in my list of creditors? What happens if i include credit agency B on my list of creditors and then after i included them they immediately sell my debt to credit agency C? Would this mean that the debt with credit agency C is not discharged because credit agency C is not on my list of creditors? Also, if you could give me some tips on possible pitfalls to declaring bankruptcy on my own, i will greatly appreciate it. What tricks will my creditors try to pull on me once they discover I will be declaring bankruptcy? Should I hire an Attorney? What value will an attorney provide that i can't provide on my own?

You SHOULD consult with an experienced bankruptcy attorney that regularly practices in the district where you would be filing. Bankruptcy is not an appropriate topic for a "Do It Yourself" project.
