when filing bankruptcy on credit card debt, does one have to file bankruptcy on all credit cards in ones name?

Yes. You must list all of your creditors in your bankruptcy schedules. Any credit card with an outstanding balance must be listed.

...and not only credit cards, but the money you owe your mother, your rent, your mortgage, your car payment, your alimony payment, your structured settlement payment, your student loans, and every other penny that goes out of your wallet in payment of any debt whatsoever. Note that not all of these debts can be discharged (student loans are a fine example of that), but you must list all your debts.
