Legal Question in Bankruptcy in New York

using power of attorney to file

i need to file bankruptcy for my mom.but i dont have power of attorney yet.is there a specific type of form that i have to use?i have already purchased a durable power of attorney form.but it dosen't give provisions for filing for bankruptcy.
