Business Name Change
I am changing the name of my business and have an outstanding loan with a bank do I need to notify them and sign new documents.

I assume you are a sole proprietorship dba under a trade name. If so, you do not have to notify them unless your loan agreement so specifies, since you are still liable for the debt.
