What type of conset form do I need for a business I am forming providing Botox Injections in a Spa setting?

I'm not exactly sure if you're asking for forms as far as starting your business, and the legal paperwork associated with that, I'm assuming you mean as far as dealing with clients directly. There is no standard release form and you could either attempt to write one up yourself or hire an attorney to write one up for you.
