I am a discloser in new york dealing with a recipient in Mass. does a New York form cover both states?

It depends on various factors. If you meet with them in NY, yes. If you meet with them only in MA, probably not. If you only meet over the phone from different states, then your chances improve of using the NY contract. Be sure to get the contract signed and sent to you before you disclose anything.
