personal guaranty
Does a personal guaranty must me be notarized by a public notary in order to be fully active or a personal signature and initials are OK?

The terms of the PG may stipulate something more but, as a general matter, a signature is sufficient. Notarization simply indicates that the signer is who they claim to be, nothing more.

You are asking what is needed to make a personal guarantee enforceable and valid.
It depends on several things, including what the person is guaranteeing, what the person is getting out of it, and other factors.
If there is enough money involved to make it worth the expense, then you should ask an attorney for advice.
