I want to register in New York as a sole proprietor. Where can I get the form required (D.B.A.) to register with the County Clerk?

Re: registering a business
You only need to file a certificate for assumed name if you conduct your business under a name other than your legal name. In any event, county clerk's office in the county your business is located would have the form. Online, try www.albanycounty.com/elected/clerk/_docs/dba.rtf for an example of the form.
