Is there a format/form that I can use to submit my own verified answer to a summons and complaint we received.

Call the clerk of the court that issued the summons to see if they have the forms you seek (small claims may; others probably won't), or, better, send the summons and verified complaint to your attorney. If you don't have an attorney, you should hire one unless this is a small claims matter (even then, having a lawyer in your corner doesn't hurt). Solos and small firms generally charge less than do large firms for the same service, if money is a factor.
To get a referral to a local attorney, check with your county bar association or the New York State Bar Association (www.nysba.org). Do this SOON; if you don't answer a summons on time, then the consequences can be severe.
