hipaa regulations

Hello, I am a physical therapist and I contract with a large agency located in NY (I am an independent contractor). Through the agency I work in a school district treating children. The agency is now requiring me to have the classroom teacher (or substitute teacher) or classroom aide sign a ''Service attendance sheet'' for each child that I treat. On this sheet, aside from the date and time of treatment, it also asks me to note the ''focus of session''- which would be my treatment focus for the session. I feel that this may be in violation of HIPAA regulations but I am not sure. The agency states it is not becasue the classroom teacher also has the child's IEP. Any information you could give me would be greatly appreciated

If it is not for the IEP, why would they wan it? Could you not simply write "physical therapy" as the focus?
