Legal Question in Civil Rights Law in New York

I own a house with my Dad. I paid for the house fully, I just had his name in case if I died then he would get it. However 8 yrs ago he had a stroke. He was home with me until 2 years ago when I had to put him in a nursing home. His right side is fully parralyzed and also he has lost his speech. I would like to remove him from the title. What would be the best way to do it. God forbid if he died what would happen to the title and ownership?
