Are there restrictions regarding age for using websites that include forms (such as medical, recommendations)? Does a person need to be at least 18 to fill out and submit forms for teen programs?

Yes, a safe bet would be to make sure that the respondent is over 18 then there is a presumption that they can enter into a contractual arrangement.
