Last Name Change

Hello I am married and i would like to change my last name only to my husbands, how would i go about that process?

Re: Last Name Change

Normally, you would have the option of changing your name when you sign the marriage license. However, if you failed to do so, you can change your name under the Civil Rights Law with a simple application to either the Supreme Court or Civil Court. You can probably retrieve the application from the clerk's office.
