Legal Question in Consumer Law in New York
birth certificate
I moved from ny to wv and when i was born my last name was spelled wrong and now i am 39 and want to get this corrected. how do i do this?

Go to the birth records office in the municipality in which you were born. Get a certified copy of the birth record. Explain that a ministerial error was made and that you want to correct that ministerial error. It should be that they will give you a form and that you can submit that form with supporting documentation that they will ask you for and you should be able to correct that without the need to go to Court to get a court order changing name.
