Affidavit of Confession of Judgement need signing?

I have agreed, verbally, to pay an outstanding hospital bill to a collection agency. They insist I sign an ''Affidavit of Confession of Judgement'' or they will proceed to attempt garnishment of wages. Do I have to sign this to avoid this action? They have accepted a payment without my returning the ''Affidavit'' but called me afterwards and threatened to proceed with further action if I don't sign and mail it in. So the question is this: What further action will occur if I don't sign this form and simply continue to pay off the debt, aprox $3850.00.?

I would not sign and continue to make the payments. Why make their job easier? You will, in essence, be giving them a judgment...who needs that? Let them go to court for it if you stop making payments.
