Checking Tenant References on Rental Applications

I would like to know what form to send to check the credit of prospective tenants. They have filled out a Rental Application, and listed several credit references. I need a generic form to send to these references.

There is no standard form. Best course to follow is to send your letterhead to each of the credit references with an explanation...something like this: You have been listed as a credit reference for (Prospective Tenant) who has submitted a tenant application. Please be so kind as to respond to the following questions and mail your responses to (Your address) using the self addressed, stamped envelope provided. Thank you.

1. What is the basis for your relationship with applicant?

( ) business

( ) personal

2. How long have you known applicant?

3. On a scale of 1 to 5, 5 being the best, how would you rank applicant's honesty?

4. On a scale of 1 to 5, 5 being the best, how would you rank applicant's financial responsibility?

5. (ANY OTHER QUESTIONS YOU CAN THINK OF THAT ARE APPROPRIATE). Don't be shy...
