doctor wants to place lein on patients pending law suit

hello

i have a client who is a doctor and has a patient who owes hima small amount of money. this patient has refferred him to his lawyer because he has a pending lawsuit which he expects to collect. the lawyer representing this patient has told us to send him a lien form and if his client agrees he will sign. he was not specific as to what form and did not want to speak further. what kind of form would he be talking about or what kind of form should we send?
