Information subpeona

I have won judgment in civil ct and debtor owes us $1700. I have filed with county & city sherrif. I want to levy bank account and was told I need an information subpeona. I have found one on blumberg legalforms. How do I without an attorney have this issued?

It has to be issued by the Court Clerk.
