How do I apply for Certificate of relief from Civil Duties?

Correct me if I'm mistaken but I think you meant to say a "Certificate of Relief from Civil Disabilities". The relief may be available based upon the type of conviction you received and the circumstances of the case. Application is made to the Judge who presided over your case.
