Legal Question in Criminal Law in New York

I received a summon for Reckless Driving when in fact it was the issuing Officer that was driving recklessly by making an improper lane change without giving any indication that he was going to do so. I was then pulled over, my documentation was obtained, and I was then issued the summons, literally. After some time, I reviewed the summons and noticed on the bottom where I says "I acknowledge receipt of this summons..." the Officer had fraudlently signed my name. The signature is no where near my signature, nor was I even asked to read and sign in the designated area. Please advise.
