Legal Question in Discrimination Law in New York

I have a son who is mentally retarted who has just started working for the YWCA ,They sent him a paper that tha title is Statewide central Register Database check,Should he fill this document out or is this a legal document? I have call the YMCA and inquired about this paper and they just told me how to fill it out .

This is nothing more than an authorization for a background check since he is working with children.
