What specifics should I include when writing to the judge I had for my 1st offense DUI case, requesting permission to respectfully leave the country for an annual family trip that has already been paid for (prior to me getting my DUI)?

This is a question that you should discuss with your lawyer. You do not want to put any information in trhe letter which would negatively iompact your case. Also, you need to determine what were the conditions of your release.
