I am completely an application for my Mother for Long Term Care Insurance. My father is deceased. To I put her down as a "Mrs" "Miss" or Ms'? Thank you.

I am sorry to hear of your father's death. Please accept my condolences.
I would advise you to ask the insurance agent.
I believe 'Mrs.' is the most correct, although non are incorrect. I think the use of 'Miss' by a senior female is inappropriate, but that's just my opinion. I was not an English major.
