My name is listed incorrectly on my 91 year old mother's power of attorney form and her health care proxy form that she had drawn up by her lawyer in 2005. She apparently listed my name as my married surname, which I had never changed when I got married in 1992. (My marriage license lists my name as hyphenated, but I never changed it. All my IDs are still in my maiden name) My sister is also listed on the forms, whose name is correct. Is this something I should have changed on my mother's documents? My mother is currently healthy, living alone since my father died in 2011.

Yes. Make sure your name and address agree with your driver's license. Get it done asap while mom still understands what she's signing.
