Depends on when, to whom, which assets, when you go into nursing home and whether you want nursing home or home care medicaid. You very likely may be disqualified. This question could only be answered with a full review of your situation.

It depends on how much you transfer and when. Lots of variables can't answer this question without a detailed analysis.

Their are look back situations that can reach back to the assets you've transfered. Additionally, there are issues relating to the other person, specifically that the assets then belong to the transfered party. You may want to consider a trust, and to speak with an elder law professional before making any decisions.
