Is there a general form to use as a music agent for a group.

There's a common form used but each arrangement is different. Here (http://www.spmlawyer.com/FormAgtMgt.html) you'll find a manager agreement that could be adapted. As an agent, however, you're required to be licensed by the Dept. of Consumer Affairs. Otherwise, you risk losing all commissions.
