Legal Question in Family Law in New York

I am a 23yr old married woman residing in queens, NY. I had a boyfriend and and had a child with him in 2005. We seperated and both went our seperate ways. I then met my current husband and pretty much after we married, my child's father stopped giving me money. My husband has no problem supporting us but now I am officially seeking custody of my child after he made a threat to take her. I am also seeking child support. I do not work and have not been employed since I married my husband in 2007. We now have our first child support hearing and they have asked me to fill out form 4-17 2006. In this form it asks me alot of questions about employment and assests, I have neither. However, my child and I are supported by my husband and we are under his health insurance. He also owns a car but thats about it. Other than providing our joint W2 for last year and his salary, what else on the form am I required to have him fill out with his information?

Your present husband's income should be excluded from the calculation of child support.
