birth certificate name change
I am 79 years old and never had a birth certificate. I applied for one last year and found out the name on it is not the name I had been using all my life. All my papers such as drivers license,marriage license, social security card, military records , etc have all been in the name I used. How do I go about changing my birth certificate? I was born in the state of Florida and I now reside in NY state. I would like to have one so I can get a passport. Thank you

Believe it or not, your situation is not unique.
Your legal name is what is on your birth certificate. In order to change your offical name to the name you have been using, you'll need to do a name change.
A name change is something many people successfully do without a lawyer. You can download the forms at no charge from the unified court system home page - www.courts.state.ny.us
