Name change

What do I need to do to change my and families last name? I am a resident of NY currently active duty air force stationed in Japan. My father was not on my birth certificate and I would like to change to his last name. Dont know the where about of mother but have contact with father. I would like to know what process I need and aproximate cost.

The process to change a name in New York is relatively simple, and not expensive. It varies, however, by whether you are a resident of New York City or not - if you're in New York City, you can do it in Civil Court or State Supreme Court, if not, you go to State Supreme Court in your home county.

It also varies by whether the individual changing a name is a minor or not.

There are some relatively simple forms to fill out, and, if children are involved, you need signatures of parents.

For one adult, the total cost, including court fees, should be under $1000 if you do it through a reasonably priced attorney.
