Changing my last name

I was considering of changing my last name to my mothers maiden name. I never had my father last name, so my sister's father gave me his. But i rather keep my mothers last name.

Changing last name is easy.

Forms are available on web. If you search answers in this site for name change, there is plenty of information.

Youy local Supreme, Civil or District courts has a "pro-se" clerk who will give you forms and instructions if you cannot find them online.
