Child care while out of the Country
I am traveling out of the country on vacation. What forms do I need to fill out so my sister can make decisions regarding my children on my behave. The children are 16 and 17.

A notarized statement that your sister is empowered to make health care and other decisions from the date of your leaving until your return should be sufficient.
The fact that she is your sister is helpful.
