Legal Question in Family Law in New York

A contract for same-sex domestic partners

I will be moving in with my fiance and would like to know if there is a legal contract for same-sex domestic partners (we reside in NY) which would allow us to make emergency medical decisions for one another as well and for health insurance and financial bank accounts, etc.

For decisions concerning money and finances (bank accounts, securities, etc.), you may draft a power of attorney. This is a simple form that you can retrieve in a stationary store.

For decisions concerning medical, you can draft a health care proxy - different than the power of attorney. A standard health care proxy form may be located on the internet.
