Need evidence from an old case for a new case

I was a party in a custody trial, during this trial a pay stub was entered as evidence. A mistrial was declared. A new relocation/custody hearing has begun (same parties and same judge). I want the pay stub from the previous trial to be admitted as evidence in the new hearing. The judge said, ''Fine, just file the proper forms.'' Does anyone know what forms or type of motion should be filed? Thanks for any help in advance!

The procedures for submitting documents "into evidence are listed in the Evidence Rules under a variety of categories, that you may review in the

Court Library, open to lawyers and litigants alike.

You may also ask the Judge's Law Asst. specifically what the Judge requires concerning the specific document you want to submit to the Court, to set you in the right direction.
