where can I obtain a notice of appearance form to submit to my ex wives lawyer ?

Your lawyer should have it. If you do not have a lawyer, get one. If you just need papers for a do-it-yourself uncontested divorce, you can get them from the Matrimonial Clerk in the Supreme Court in your county.
