Giving Power of Attorney for Child Visiting Relatives

My 15 year old daughter is visiting my brother's family and my parents (two households, same area) in Florida. I live in NYC. She has to see a doctor for a rash, but I did not think to send power of attorney.

1. If I am downloading a form for childcare power of attorney, do I use Florida form or NY Form?

2. Does her father need to sign (we are divorced and I am custodial parent, he lives somewhere else)

3. Finally, will a fax copy be sufficient for doctor to treat her, or will only original do?

If you are in NY, you use NY POA. Copy of same is sufficient to send /fax to doctor. Your signature alone is sufficient as well.

I suggest using the Florida form, since it will be used in that State and people there are more familiar with it. It needs to be signed and NOTARIZED by you. Your authorization should be sufficient without the father's being added, since you are the custodial parent. Some doctors and clinics may insist on an original, so I would send the document by overnight delivery service (Fed Ex, DSL, UPS, etc.). Please call if there are complications. Or just to clarify what we discussed here.
