Legal Question in Family Law in New York

Hello,

My boyfriend (age 25, bronx, NY) has wanted to change his last name but is very confused where to start.

His family (grandmother) wants to legal adopt him as well as change his name to theirs. Currently he has is mother's maiden name. His mother had him before she was married and then died when he was very young leaving him with his grandmother and father (not biological). As far as I understand, he was never legally their son/grandson. He would like to change his name to his grandmother's last name and legally be part of the family.

He also gets benefits, like insurance, under his biological father's name (not really sure how) who has a totally different last name. So he doesn't want to complicate things and lose his benefits. He has a disease so he really counts on those benefits.

Any advice on how to start?

Also, any idea how much this process could cost?

Under NY law, if you are an adult, you may petition the court to change your name. You do not need to be adopted. If you go to the pro se office at the Supreme Court in the county where you are located, you will receive forms that will help you with your petition.
