Legal Question in Family Law in New York

My husband and I would like to file for a legal separation. Which office must we visit to get the forms. Also, I am not sure but after we complete the forms and sign in front of a notary public, do we just have to file at the County Clerk office?

There is no particular "form." A separation agreement is drafted by the parties. If you need, help, I suggest you consult a lawyer.
