Income Execution Form

After receiving Judgment for my Divorce, (I represented myself), I acted upon receiving my monies which was granted to me. I filled out the necessary papers which the Marshal requested. But a month later I still did not receive any monies and the Marshal has informed me that I need an "Income Execution Form". I picked up a Bloomberg Form T439 but need advice on how to fill it out. Can you please help me in this matter.

The marshall's office should help you with them. Thats why you pay them a percentage of whatever you recover.
