legal name change for children with seperate parents

last year me and my fiance broke up and i now have custody of my daughter. my ex has legal visitations to her, and my daughter has his last name. do i need his consent to give her my last name instead of his??

Re: legal name change for children with seperate parents

In order to legally change her name, you will have to go to surrogates court. Contact the surrogates court in your county for the paperwork. There is no assurance the judge will grant your request.
