Suggest that you consider petitioning both Criminal & Civil Court for Fraud, and other crimes; and for a Restraining Order and other types of Injunctions against the father, to prevent him from using your child's name (whatever it may be), now or in the future, for fraudulent and illegal purposes.

Please note, a name change alone will not prevent or avoid the abuse you describe. Nor will a Court Order that is not "Published" to any all parties or agencies that may become the victim of your Ex's schemes.
