Legal Question in Family Law in New York

I live in New York State. Currently, my children's father and I share custody of our children with a 60/40 split schedule (in my favor). This was done through family court, so there is an order in place.

He is moving out of state. We agree that I should now have sole custody and have worked out a visitation schedule. What is the best way to get the order changed? Is there anyway to get a new order in place without filing a petition? Can we file a joint petition to show we are in complete agreement (skipping the lawyers-- especially the one for the kids)? This is a formality, so we'd rather not make it weird for the kids by having them meet with an attorney.

The only way to do this right is to file a petition and appear in court.
