Living will/power of attorney

What is the best living will form, for a single parent that has an adult disable child, who cannot fend for themselves, and need legal gaurdian, in the event of the parents death. For the state of New York City.

In your situation, you need an attorney to prepare a living will and health care proxy along with a standard will appointing a person or organization as legal guardian for the disabled child. The over the counter forms most likely will be too difficult to modify for yor purposes and I do not recommend them due to there severe limitations in complicated matters. You should contact an attorney.
