Legal Question in Family Law in New York

quick claim deed

my ex has refused to sign a quick claim deed that she was court ordered to in the divorce decree. i want to file in the supreme court the form i need to get the judge to order her because she is in violation. what form do i need? i found two online. they were contempt of court and an affadavit in support of motion for contempt. is it one of these? if not do you know which one and where i can download it?

You will have to file an order to show casue and ask for the specific relief. The pro se clerk at the court might be able to help you.
