relief from a judgement

Do you have a sample of a motion called ''relief from a judgement'' or to ''vacate an order'' CPLR 5015 so I can make this motion in court?. I feel my attorney misguided or misled me.

You are going to need more than a form to vacate a judgment. Most likely, even if your attorney misguided you you will need to appeal it, assuming you still have time to do so.
