Legal Question in Family Law in New York

I am a resident of New York State. I am 33 years old and have not had contact with the person listed as my father on my birth certificate for almost 20 years. I would like to have the person I consider to be my father legally adopt me. I want to know exactly what forms need to be filled out and submitted - there are over 75 I've come across on NY State websites, but nothing specifically for adult adoption. Any help is very appreciated.
