Who is responsible for enforcing legal rulings?

My fiance was awarded physical custody of his 6 year old son. In the court papers there are stipulations to the mothers visitation, ie: house must be clean, child must be fed, clothes changed, etc. She recently moved and did not tell us where she moved to, we found out from the child. The child also told us the apartment was ''crummy'', there were bees, a broken window, and they cannot drink the water. My fiance called to ask if he could come see the apartment. The mothers boyfriend of only about 3 months called back and said no to seeing the apartment. He was loud, rude, fighting, and yelling. The mother has a history of horrible housekeeping, which was a major factor in court. My fiance is trying to keep the peace, but is worried about the living conditions of his son. They are not cooperating and we feel the mother is not holding up her end of the agreement. Who is responsible for ensuring that she is keeping up on her end of the arrangement? The police were no help. Will we need to go back to court, and if so, what kind of case would we file? Any information would be helpful.

You will need to go back to court and have witnesses describe the condition of the apartment, pictures would be helpful.

You're 26 years old, your daughter is living not with her father but with her father's mother and you're not happy with the way her grandmother is raising her? Your daughter's cold to you?
